    Welcome to the Launch Housing Rental Readiness Referral Form.

    Clients must be referred to the Rental Readiness Program by a service provider, including IAP, to ensure appropriate assessment. Self-referrals will not be accepted and will be directed to an appropriate service provider.
    The Rental Readiness Program is an education program that assists people to gain a greater knowledge and skill level of how to search, apply and sustain a private rental tenancy.

    Please complete the Pre-Referral Conditions section below. You must answer each question to be able to proceed with the application.

    The form may take between 15 and 20 minutes to complete. You can use a mobile phone, tablet or desktop computer to complete this form.

  • Pre-Referral Checks

    The referral application can only proceed if all the below checks are met.
  • Has the client identified private rental as a goal? The client must identify private rental as a goal.*
  • Those who secure private rental will most likely be removed from the Victorian Housing Register priority list. Is your client willing to be removed from the priority list for the VHR?*
  • Do you have copies of the client's ID? Two forms of ID are needed for rental applications.*
  • Does the client have a housing support worker involved? Rental Readiness referrals will not be accepted when the client has a current housing support worker.*
  • Do you have the personal details of all the adults that will be part of the rental applications along with their income details?*
  • Does your client provide consent for Launch Housing to collect, use and share their personal information for the purposes of referral to the Rental Readiness program?*
  • Thank you for your interest in applying, however, the client is ineligible for the Private Rental Assistance Program through Launch Housing.

    The client:

    • Cannot identify public housing as a goal
    • Must have two forms of valid ID (including photo)
    • Cannot have a housing worker involved
    • Must have income to support a private rental tenancy
    • Must be able to self-manage their private rental tenancy.
    • Must be able to provide details of the household members they intend to reside with.
    • Must agree to share their details with Launch Housing to apply for PRAP.

  • Hi there,

    Thank you for your interest in applying, however, the client is outside of our catchment area and therefore ineligible to apply through Launch Housing for the Private Rental Assistance Program. Please go to the Victorian Department of Health and Human Services website below to find the client's local PRAP service.
